NAPNAP’s 36th Annual Conference, on Pediatric Health Care, will be held March 11-14, 2015, in Las Vegas, NV.  NAPNAP’s Conference Planning Committee is now accepting abstracts for Clinical Roundup Presentations, Research Podium/Posters, Practice Innovations Posters, and Intensive Workshops (IWS.)

Children's Hospitals Receive Holiday Gift Bags | Melnic Consulting Group


One of the true joys of the holiday season is the opportunity to spread happiness to others.  Melnic Consulting Group had the pleasure of spreading holiday cheer to children, families, and pediatric staff at leading children’s hospitals around the country. The Melnic team filled holiday bags with over 3,000 toys, games, crafts, stickers, and novelties that were delivered to children’s hospitals to help bring smiles to the patients and the family members of those undergoing care at this time of year.

Yummy snacks were also sent to the fabulous pediatric staff at children’s hospitals across the country to help them through the holidays and to show our unending admiration and gratitude for all that they do.

TEXT2COPE

Lisa Militello, Board Certified Pediatric Nurse Practitioner and current doctoral candidate, worked in collaboration with expert faculty to develop the TEXT2COPE mobile messaging program for her dissertation. She is currently looking to collaborate with primary care clinics that serve families with young children in the Ann Arbor/Detroit areas.

TEXT2COPE Program at a Glance:
  • For parents of overweight or obese 3-5 year olds (per CDC guidelines)
  • In collaboration with primary care providers/office sites
  • Goal:  To test the acceptability, feasibility, and effects of an intervention aimed to promote healthy lifestyle behaviors
  • Intervention (7 weeks) (delivered by Lisa Militello, CPNP)
  • Components
    • Education (nutrition, physical activity, screen time, label reading, portion size)
    • Cognitive behavior skills building (problem solving, goal setting, overcoming barriers, communication, habit formation)

Meeting ACO Quality Measures
While ACO measures have not been fully integrated into pediatric populations, TEXT2COPE Program facilitates understanding current ACO quality measures.
The ACO quality measures align with those used in other CMS quality programs, such as the Physician Quality Reporting System and the Electronic Health Record (EHR) Incentive Programs. The ACO quality measures also align with the National Quality Strategy and other HHS priorities, such as the Million Hearts Initiative. In developing the final rule, CMS listened to industry concerns about focusing more on outcomes and considered a broad array of measures that would help to assess an ACO's success in delivering high- quality health care at both the individual and population levels.

PICU Pediatric Nurse Practitioners Taking Call

Children’s hospitals across the country continue to adapt to meet the needs of the ever changing structure and demands of our nation’s healthcare.  Many of the PICUs nationwide are moving to 24-7 coverage because of the changes in resident/fellow hours, and the subject of Nurse Practitioners taking call has recently become a hot topic.  Many NPs who are unfamiliar with the changing structures have questions about what is now (or soon could be) expected of them.  Some questions include: Are NPs taking call from within the hospital?  Are NPs being asked to take call from home?  How are NPs being compensated for taking call?

As the leader in Pediatric Nurse Placement, Melnic Consulting Group ensures that all of their nursing candidates receive the best salary and benefits that appropriately correlate with the specifics of each job.  Without the proper knowledge of contracts, job candidates can easily overlook essential elements that advanced practice nurses are entitled to receive.  As the landscape of our healthcare system rapidly changes, it is also necessary to stay current on all employment aspects such as salary, expectations, scope of practice, benefits, insurance, etc.   

To gather insight on Pediatric Nurse Practitioners taking call, Jill Gilliland, President of Melnic, reached out to highly respected Pediatric Nurse Practitioner, Judy Verger, currently working in the PICU at Children’s Hospital of Philadelphia.  In response, Judy provided the following information:  “In the PICU at CHOP, we work all holidays (one summer, one winter.)  Nurse Practitioners get credit for the holiday if they work the night before the holiday, and the night and day of that holiday.  Off shift is not compensated for separately.  The reality is that it's part of working in an ICU, and at CHOP we do build that into the NP’s salary offer.  We require no more than an average of 1 weekend in a 4 week block, no more than 3 nights in a week, and no more than 9 nights in a 12 week block.  If our PNPs want to do more nights and weekends that is fine, but it is not mandated. They are also free to switch but they must do a minimum of 2 nights and 4 days in a 12 week block.”

Some children’s hospitals are currently hiring Pediatric Nurse Practitioners to work only nights or weekends. There are Nurse Practitioners who love that schedule, but they are few and far between. Due to the uniqueness of the schedule, it is important to keep those NPs engaged as part of the team.  While Melnic Consulting Group has not yet come across a job request for Pediatric Nurse Practitioners to take call from home, there are a few facilities that may already be testing it out.  This is a complicated process, and requires a well-defined plan and accountability structure. 

From nationwide salary research, in general, salaries for Acute Care Pediatric Nurse Practitioners in the PICU with rotating shifts are around $90,000.  Salaries can range from $85,000, on the very low end, to $100,400, on the very high end.  Regarding compensation, most hospitals fall in line with the feedback provided by Judy Verger, from Children’s Hospital of Philadelphia.  Melnic Consulting Group has worked with one individual hospital that offers a 10% call differential.  In the end, Pediatric Nurse Practitioners who are either taking call, or working a rotating shift, should be and generally are compensated with a higher salary.

Age Parameters for APRNs

Advanced Practice Registered Nurses, or APRNs, are a highly valued and integral part of our nation’s health care system.  The classification of an APRN includes certified registered nurse anesthetists, certified nurse-midwives, clinical nurse specialists and certified nurse practitioners.  As the demand for APRNs rapidly increases, the education, accreditation, certification, licensure, and the roles of APRNs continue to evolve as well.    The LACE Network recently released a statement addressing Age Parameters for APRNs.  In this statement, “the Consensus Model for APRN Regulation advocates for services and care to be defined by patient needs. Therefore, a rigid establishment of population age parameters is not in the best interest of patients.   The definition of a population identified by specific age ranges may create barriers and limit access to care for patients with specific needs or health conditions.” 

As our nation approaches a significant physician shortage, APRNs are vital to the solution.  This is an important step in expanding patient access to APRNs and reducing the health care barriers to those in need.

Children's Hospital Growth, Denver

Children’s Hospital in Denver

The Children’s Hospital in Denver has broken ground on a new ten-story tower that will add 124 beds upon completion with two floors of additional space to eventually increase patient capacity in the tower to 500 beds. One of the largest children’s hospitals in the country and rapidly growing beyond expectations, the construction on this 350,000 square foot expansion at a cost of $230 million was started in July of 2010. It will also increase space for cancer treatment, advanced maternal fetal medicine, cardiac and rehabilitation departments. The hospital currently serves approximately 150,000 pediatric patients annually as they work to provide the best possible health care outcomes for children in the region.

Accountable Care Organizations

Kansas Health Institute News Service examines accountable care organizations (ACOs): "In an effort to combat the lack of coordination that is widely recognized as a major shortcoming of the current health system, the new federal health reform law includes provisions intended to encourage wider use of Accountable Care Organizations or ACOs." Via Christi Health and Advantra Total Care will launch an "ACO-like model" in Kansas. Under the plan, Via Christi Vice President Janell Moerer said, enrollees "will undergo a 'wellness screening' that will be used to create a plan for their 'getting and staying healthy'" and be assigned a "navigator." Other Kansas groups are "also are assembling ACO-like plans" including ProviDRs Care Network (Ranney, 11/29).

Mobile Medicine Nurse Pracititoner

Bellingham Nurse Practitioner Bicycles to Treat her Patients at Their Homes and Workplaces
Bellingham Herald (WA) (10/18/10) Kahn, Dean

In Bellingham, Wash., nurse practitioner Jody Hoppis started the Mobile Medicine service, which involves visiting patients at their homes and workplaces by bicycle, using a specially designed bike trailer to carry supplies. Hoppis says this service allows patients to save on the time, expense, and hassle of getting to a clinic by making her available by phone or email. Hoppis says she keeps her overhead low, allowing her to spend up to an hour with patients. As a nurse practitioner, she performs physical examinations, diagnoses and treats illnesses and chronic health problems, prescribes medications and therapies, orders diagnostic tests, and refers patients to specialists, if needed. Nearly all of Hoppis' calls are made by bike, unless weather or distance is a concern.

Dinners out gave us the opportunity to chat with our friends who were Pediatric Nurse Practitioners and enjoy the great food and skyline of Chicago. Here are some of the conference highlights:
- NAPNAP has worked hard to educate and encourage members to embrace advocacy and the issues that affect their scope of practice, reimbursement, autonomy and most of all the quality and access of heath care for children.
- NAPNAP had a number of acute care topics at the conference and plans to increase the topics to at least one acute care topic per session next year.
- The Acute Care SIG Group at NAPNAP includes Pediatric Nurse Practitioners working in inpatient units and is not limited to those people working in ICU’s.
- NAPNAP had the largest attendance ever at the Chicago 2010 Conference including a large number of Pediatric Nurse Practitioner students.
- We all enjoyed Melnic Consulting Group’s Wine and Cheese Event on Thursday night to thank PNP’s and Professors of PNP programs for all of their hard work and dedication.
- NAPNAP has worked hard on the “Healthy Eating and Activity Together" (HEAT) Program to improve child health through culturally appropriate interventions that enhance the family's ability to achieve the ideal balance between nutrition and physical activity to support optimal growth and wellness.

NAPNAP Fly In Day Washington DC

Fly In Day Washington DC Get's a Visit From NAPNAP

I am thrilled to be participating in the annual NAPNAP Fly In on March 15th and 16th, 2010. This will be an outstanding learning experience.

NAPNAP has done a great job organizing a day for nurse practitioners and those affiliated with the nursing healthcare industry to fly in and meet their congress person.

The guiding principles of NAPNAP’s health policy agenda are to:

-Improve the health of children through federal legislation and regulation so that all children have access to comprehensive, continuous, family-centered coordinated, compassionate, and culturally effective care, including mental health services, promotes healthy lifestyles
-Remove barriers to pediatric nurse practitioner practice in all health care settings
-Increased commitment to national and grassroots advocacy and dedication to supporting members’ development as advocates

NAPNAP’s Health Policy Committee has identified the following goals for focused advocacy and strategic partnerships that support NAPNAP’s mission and respond to important contemporary health policy issues:

Goal #1 Improve children’s health through support and direct advocacy on the following issues:

-Preventing childhood overweight and obesity through policies that support strategies to promote healthy nutrition and physical activity levels.
-Increasing the awareness of pediatric pain management issues.
-Ensuring that all children have access to a pediatric primary care provider such as a nurse practitioner or a physician.
-Raising awareness about children’s mental health and emotional problems and the great need for access to mental health services.
-Encouraging development of more quality indicators focused on the health care of children.
-Supporting full funding of Medicaid and SCHIP programs.
-Increasing awareness of the benefits of breastfeeding
-Increasing vaccination rates among children through improved access and education
-Promoting oral health, caries screening and oral health coverage

Goal #2 Promote access to Nurse Practitioners through support and direct advocacy on the following issues:

-Recognizing nurse practitioners as leaders of medical homes.
-Promoting increased funding for advanced practice registered nurse education and development of faculty.
-Expanding coverage and reimbursement by governmental and commercial payers for services provided by nurse practitioners.
-Ensuring nurse practitioners are permitted to participate in any federal grant programs aimed at increasing the use of health information technology and electronic medical records.
-Promoting access to a pediatric primary care provider such as a nurse practitioner or a physician for all children.
