Advanced Practice Provider Scope of Practice in the Emergency Department

Advanced Practice Provider (APP) Scope of Practice in the Emergency Department

With the changing climate of health care in the U.S., advanced practice providers are assuming more responsibility for the direct care and clinical management of acutely ill or injured infants, children, teens and young adults in the emergency setting than ever before.  These providers are experienced in urgent/acute care medicine and many are nationally certified nurse practitioners or clinical nurse specialists who practice in hospital clinic emergency departments.  They evaluate, stabilize, treat and care for pediatric trauma patients through histories, assessments, physical examinations, ordering and performing diagnostic tests and procedures, prescribing medications and developing treatment plans.

Pediatric Nurse Practitioners Find New Opportunities in Urgent Care Facilities

As the need for more primary care providers increases due to the continuing shortage of medical doctors and the changes in our health care system, many nurse practitioners are finding new opportunities, particularly in the area of urgent care.  Some medical centers are moving toward staffing their urgent care facilities with teams of nurse practitioners, skilled clinicians who provide immediate medical services and out-patient care for those with acute and chronic illnesses and injuries.  Many members of these teams have completed advanced coursework and have clinical experience in pediatric urgent care.  They work in fast paced, high volume settings where the average census could be as high as 150-200 in a day of patients with urgent but not life threatening conditions.  Rotating shifts provide optimum urgent care coverage, and this increasingly popular team approach frees up doctors to treat the more seriously ill or injured patients in the ER while giving nurse practitioners new leadership opportunities in advanced nursing practice.

Children’s Emergency Care Evaluation

North Carolina has made it a priority to evaluate the care of children in their regional emergency departments.  According to an article published by the Winston-Salem Journal, a $900,000 federal grant was issued to aid in the evaluation and 13 NC hospitals will participate in the study.  The effort is led by the Childress Institute for Pediatric Trauma at Wake Forest Baptist Medical Center. 

A lack of appropriate equipment, training, and treatment has been identified when tending to the care of children outside of pediatric trauma centers or pediatric emergency departments.  Dr. Wayne Meredith, executive director of the Childress Institute says that “in too many instances, children are treated like small adults in non-pediatric trauma centers or in the children's units of adult hospitals.”

Every year millions of children are brought to community hospital emergency departments rather than children’s hospitals.  Children require unique and special care, especially in a life-threatening emergency situation.  The goal of the evaluation is to develop standardized guidelines and resources that are then implemented in emergency departments that tend to the care of children in emergency circumstances.
